Every author is a bit different in how a begin their story, some just start writing, others write very short chapters, and others write an outline. No matter how you get started, sooner or later you're going to have to have an outline to make sure that your story runs true to itself.

It really is as simple as that, you can add and subtract chapters as you go, you can even add characters, but if you don't have an outline, you're going to get lost in your story.
